Case 17 History ---- The patient was a 63yo female with a past medical history of hypertension, seronegative rheumatoid arthritis, and psoriasis who presented with left-sided sensory loss and a progressively enlarging medullary lesion on imaging which were both refractory to steroids and plasmapheresis. Operative procedure: Spinal cord biopsy. Following the biopsy she developed chronic respiratory failure and ventilator dependence, quadriplegia, and autonomic instability. She had a chronic indwelling Foley and developed a complicated urinary tract infection, a stage IV sacral decubitus ulcer, septic shock and passed away. ---- 17A1-3 These axial and sagittal T2-weighted scans show patchy hyperintensity of the medulla extending into the rostral spinal cord.
