Case 5 History ---- The patient was a 54-year-old man with a history of multiple sclerosis, seizures since childhood, chronic obstructive pulmonary disease, and coronary artery disease status post myocardial infarction in 2005. The patient has been residing at an assisted living facility for several years, as he had seizures every one to two months and over time his mental status had been in decline. His multiple sclerosis had also been slowly progressing. In March 2011, he noted increased somnolence, speech difficulties, memory and recognition deficits, and ataxia. Per the assisted living center, the patient had been sick with respiratory symptoms and was found unresponsive in his bed.
