Case 2 History ---- This is a 64 year old male with a long history of multiple sclerosis. He was first diagnosed in 1998 with symptoms which included stiffness, back pain and gait imbalance. This diagnosis was supported by MRI and a lumbar puncture which showed positive oligoclonal bands (OCBs). He had optic neuritis of the left eye in 2001. His most recent symptoms included weakness/ clumsiness, trouble walking, fatigue, tremors, cramps or spasms, numbness/ tingling, neurogenic bladder, constipation, sexual problems, depression, and anxiety, and metastatic melanoma. He also has a previous history of melanoma on his face which was discovered a few years prior to expiration. He recently had noticed a lump in his right upper abdominal wall in early May of 2016. He received a CT scan workup which revealed multiple pulmonary nodules, an enhancing lesion in the urinary bladder and metastasis to the brain. His terminal course is not available to us.
