Case 3 History ---- This patient was a 54-year-old man with a history of multiple sclerosis, seizures since childhood with tongue biting and bowel and bladder incontinence, chronic obstructive pulmonary disease, and coronary artery disease status post myocardial infarction in 2005. The patient had been residing at an assisted living facility for several years, as he had seizures every one to two months and over time his mental status had been in decline. MS residua had also accumulated with time. In March 2011, she noted increased somnolence, speech difficulties, memory and recognition deficits and ataxia. He had been sick with respiratory symptoms and was found unresponsive in his bed.
