Case 2 History ---- The patient was a 32 year old woman with a history of bipolar disorder, multiple sclerosis, morbid obesity, obstructive sleep apnea, asthma, lupus, rheumatoid arthritis, depression, borderline personality disorder, left leg deep venous thrombosis, and seizures (refractory to medical therapy), chronic left leg ulcers, as well as multiple suicide attempts including overdoses with aspirin as well as mixed prescription medications. Multiple sclerosis was diagnosed in 2006 with supportive evidence from multiple MRIs showing fluctuating lesions in the periventricular white matter as well as the corpus callosum. In addition, there were oligoclonal bands present within the CSF. She was screened for NMO-IgG antibodies but was negative. A few days before her death, the patient developed a high fever and was being treated with acetaminophen. She was transported to the emergency room from a long term care facility after having a witnessed seizure before becoming unresponsive. Upon arrival, emergency medical personnel found the patient in cardiac asystole. She underwent endotracheal intubation and advanced cardiac life support was initiated but was unsuccessful.
