3A0 Case 3 History
Case 3 ---- The patient was a 63 year old woman who developed MS in 1964 when she had difficulty walking, which had followed a prior history of spontaneous blindness in the right eye (many years) and years of poor vision in the left eye. In 1968 she was confined to a wheelchair and in 1973 was completely bedfast, developing multiple bedsores. Her mental status was normal; extraocular movements were full without any cranial nerve abnormalities. She had no voluntary movement in the lower extremities with flaccid legs and moderate weakness of the upper extremities. Sensation to pinprick was decreased below T4 and vibratory sensation was absent in the lower extremities and diminished in the upper extremities. Plantar reflexes were strongly extensor. The patient was seen 6 months before her death with CNS signs of marked confusion and worsening of vision in the left eye. Additional details about her terminal course are unknown. Her death in 1978 was 14+ years after her initial diagnosis.
