Case 1 History ---- This 35 year old patient was six weeks postpartum when she presented with a two week history of worsening left upper extremity weakness. Clinical concern was for possible cerebral infarction, carotid dissection, and, particularly, favored a primary CNS malignancy. MRI examination showed an irregular enhancing 2.4 x 2 x 3.4 cm mass lesion centered in the precentral gyrus on the right. The differential diagnosis included glioma, and, less likely, metastatic disease. Additional diagnostic possibilities included tumefactive multiple sclerosis, lymphoma, or sarcoidosis. Approximately one year later the patient developed a refractory seizure disorder in the absence of change in the post-operative MRI which has remained unchanged for 15 years.
