Case 4 History ---- The decedent was a 70-year-old woman with a history of neuromyelitis optica (Ig-NMO positive, "high" levels by assay) and prolonged immobilization with episodes of autonomic instability. EMS was called to the patient's home by her home nurse who stated that the patient developed hypotension and altered sensorium at home. On arrival to the ER, the patient was unable to provide adequate history and then developed bradycardia which progressed to pulseless electrical activity from which she did not recover. At autopsy a large pulmonary embolus was discovered.
