A small stereotactic biopsy generated a minute amount of white matter: 1A1,2 The pallor of the H&E stained white matter suggests myelin loss. A typical active demyelinative process is rarely this devoid of macrophages and inflammatory elements, which, in this case, was attributed to the effect of pretreatment with high dose steroids and that the lesion may have had a long history before biopsy (H&E).
