Case 1 History ---- This 27 month old 3720 gram female was the 42 week gestation product of a G3P2Ab0 37 year old mother whose pregnancy was complicated by pyelonephritis and viral illnesses at 81/2, 10 and 41 weeks of gestation. Decreased motility of the fetus was noted during the last week of gestation. Labor was induced due to the length of gestation and was complicated by decreased fetal heart rate. Scalp pH 2 hours before birth was 7.33. Apgars were 6 and 8 at 1 and 5 min. At 9 hours of life the patient was noted to be lethargic and hypotonic prompting a septic work-up. The LP was traumatic and revealed CSF fluid with no signs of infection but with an elevated protein content. On day of life number 2 the patient underwent cranial ultrasound which was abnormal with echoes in the periventricular areas. On day of life number 3 the CSF cultures were negative. Because the patient continued to be lethargic, hypotonic and have a poor suck reflex the LP was repeated and revealed increased protein, normal glucose and increased white blood cells. The patient died suddenly at 27 months of age.
