In most cases of fat emboli the white matter is selectively involved by punctate hemorrhages and perivascular infarction and the cortex, which doesn’t typically develop hemorrhages, may have scattered occluded microscopic vessels. In this case there has been an apparent impressive shower of fat emboli occluding numerous microscopic vessels, likely the reason for the extent of gray matter infarction in this case. (H&E)
