Case 2 History ---- This 83-year-old man was a participant in the NIH Alzheimer's Disease Research Initiative. His memory impairment began in 2004 (then his Clinical Dementia Rating was 0.5, mild dementia) and was slowly progressive reaching CDR3 (severe dementia) at the time of expiration 7 years later. He carried a diagnosis of dementia of the Alzheimer type. A malignant melanoma was removed from the face in January 2011. He also fell in January, 2011, fracturing his hip; and underwent surgical repair. He subsequently developed a urinary tract infection and expired in January 2011.
