The major finding of this autopsy is the presence of diffuse global ischemic injury, such as laminar cortical necrosis and multifocal cerebellar Purkinje cell loss. These changes are subacute in nature, consistent with patient's clinical history of hypoxic/ ischemic perioperative injury. The sellar region contains subacute blood clots with reactive change; however, there was no evidence of residual tumor. ---- 7A1,2 The specimen is the dorsal view of the suprasellar region showing the intact pituitary stalk (7A1) and the inferior aspect of the diaphragma sella representing the previous location of the pituitary adenoma (7A2).
