Case 18 History ---- The patient is a 41-year-old woman presenting with a 3.9 cm sellar mass. Operative procedure: Endoscopic endonasal transsphenoidal surgery for tumor resection. ---- 18A1-6 H&E stained sections show a solid proliferation of poorly differentiated malignant cells with irregular nuclear membranes, vesicular chromatin, macronucleoli, and a small amount of amphophilic/basophilic cytoplasm with focal perinuclear clearing. Rhabdoid appearance, short spindled phenotype, and stag-horn vasculature are noted focally. Mitoses and karyorrhectic debris are abundant. Large areas of necrosis are appreciated within the tumor, including bland "infarct-like" appearance. The tumor extensively invades the sinonasal mucosa and adenohypophysis).
