1A0 Case 1 History
Case 1 History ---- The patient is a 19-year-old man with a history of new onset seizure activity while traveling with friends in Florida in September 2022. A head CT was done at that time and was notable for left temporal lobe calcifications and surrounding edema for which MRI was recommended. MRI further characterized a multilobulated left temporal lobe mass with calcifications and hemosiderin staining, overall consistent with primary brain tumor. He was initiated on Keppra 1000 mg BID. Once he returned home to Missouri the tumor was resected in January 2023 with no interval seizure activity and eventually a diagnosis of left temporal lobe CNS tumor with CREBBP::BCORL1 fusion, currently unclassified by the 2021 WHO CNS Tumor Bluebook. His postoperative course was uncomplicated and he was discharged home without focal neurologic deficits. He received postoperative radiation and was subsequently referred for further management. He was seen in May 2023 who recommended evaluation by neurosurgery for potential additional resection which occurred 7 months following his initial resection and chemoradiation.
