Case 1 History ---- The patient was a 4 year old female who had biopsy proven medulloblastoma in 10/92, 16 months prior to her death. Subsequently she received chemotherapy and craniospinal radiation therapy. Chemotherapy was continued for the first year of her diagnosis. An MRI in 12/93 was unremarkable. Four months later in the beginning of 3/94 she re-presented with encephalopathic changes, emesis and became unresponsive within 24 hours. An MRI was consistent with recurrent tumor and showed significant meningeal enhancement. CSF cytology was initially negative; however, a second cytologic examination a short time later showed clumps of medulloblastoma cells. Blood chemistry showed decreased sodium consistent with the syndrome of inappropriate ADH secretion. One blood culture had been positive for multiple species of bacteria. The child was discharged home after initial treatment and died in March 1994.
