Higher magnification of image #2B4. (H&E) ---- Not Shown: Synaptophysin demonstrated dot-like cytoplasmic positivity in most of the tumor cells. GFAP highlights entrapped glial processes but is largely negative in the tumor cells. P53 is negative in the tumor nuclei. The histologic and immunohistochemical features are consistent with recurrent medulloblastoma, WHO grade IV. The current tumor lacks desmoplastic features but instead has more anaplastic cytology. ---- Neuro Final Diagnosis: Medulloblastoma, WHO grade IV, with widespread intracranial dissemination involving the right basal ganglion, right temporal lobe, thalamus, right occipital lobe, corpus callosum, midbrain, pons, medulla, and cerebellum
