Higher magnification of image #2L2. (myogenin IHC) ---- Comment: The morphological, molecular, and immunohistochemical features of this tumor are consistent with an large cell/anaplastic medulloblastoma with myoblastic differentiation (WHO Grade IV). This tumor has an unusually high level of myoblastic differentiation, but much of this morphology is that of rhabdoid elements with rarer cells with cross striations. This finding along with the anaplastic appearance of the rest of the tumor raises the possibility of a transdifferentiation of immature elements.
