Comment: The morphologic and immunohistochemical features are consistent with an astrocytic neoplasm, showing some features resembling pilomyxoid astrocytoma, while other areas resemble conventional pilocytic astrocytoma, WHO grade I. Such cases of mixed morphology or intermediate features have been described previously, although the clinical significance in such cases is unclear. Based on the imaging and intraoperative gross findings in this case suggesting CSF seeding, this particular example seems to have the more aggressive biologic potential of pilomyxoid astrocytomas in general. Nonetheless, there is little clinical experience with such cases at this time and some examples have demonstrated "maturation" towards conventional pilocytic astrocytoma over time.
