There is widespread immunoreactivity for epithelial membrane antigen (EMA), displaying a cytoplasmic dot-like pattern with occasional membrane staining as well. (EMA IHC) ---- Other Immunohistochemistry (not shown): Neurofilament protein shows only focal immunoreactive axons of the periphery of the lesion consistent with a solid growth pattern. The CD99 stain is positive in a dot-like fashion, as well as staining nucleoli. The Ki-67 labeling index is high, reaching up to 15.2%. ---- Comment: The morphologic and immunohistochemical features are consistent with a diagnosis of anaplastic ependymoma with clear cell features, WHO grade 3. The clear cell variant of ependymoma is often supratentorial, cystic, and associated with anaplastic features, as was seen in this case.
