An immunostain for GFAP is strongly and diffusely positive. ---- Not shown: Immunostains for both epithelial membrane antigen (EMA) and D240 (podoplanin) show cytoplasmic granular and dot-like positivity in a subset of cells. A stain for collagen IV highlights vasculature basement membrane but does not surround individual tumor cells. ---- Overall, given the patient's history of ependymoma in the left occipital region, status post resection x 3, and radiographic evidence of multiple bony metastasis, combined with the histomorphologic and immunohistochemical features are consistent with metastatic ependymoma, WHO grade 3.
