Punctate synaptophysin can be seen in some ependymomas. (SYN IHC) ---- Not shown: INI-1 is retained. Neurofilament highlights a solid growth pattern of the tumor. FISH at Cincinnati Children's Hospital shows evidence of RELA rearrangement. Focal chromogranin punctate deposits are seen. ---- Comment: The overall findings are most consistent with an ependymoma, RELA fusion-positive, WHO grade 3. The presence of synaptophysin and chromogranin reactivity is relatively unusual in the ependymoma. However, neuronal differentiation can sometimes be seen in childhood supratentorial ependymomas (PMID: 20615923).
