Case 3 History ---- The patient was a 59 year old female with a history of early stage breast cancer s/p surgery, chemotherapy and radiotherapy who developed acute onset of left facial twitching, expressive aphasia and some numbness in her left upper extremity while on vacation. CT and MRI demonstrated a 3 x 3.5 cm central frontal lobe mass. There was no evidence of metastatic disease. Resection of the neoplasm showed a IDH wild type glioblastoma grade IV (pre-WHO 2021 CNS). Subsequent testing showed no gene methylation. ---- 3A1-3 H&E stained sections of the tumor show fragments of high-grade diffuse glioma with giant cell features. The tumor cells have atypical hyperchromatic nuclei, occasional prominent nucleoli, and abundant eosinophilic cytoplasm.
