Case 13 History ---- The patient was a 39 year old man who was diagnosed with a low grade glioma in June of 2007. A year later he recently underwent a second surgery for evidence of tumor recurrence. ---- 13A1,2 This is a highly cellular and markedly necrotic neoplasm composed predominantly of primitive appearing cells. There is moderate nuclear pleomorphism, with the majority of nuclei being oval and hyperchromatic with minimal discernible cytoplasm. In some areas, there are enlarged bizarre nuclei with increased cytoplasm, consistent with an anaplastic component. The mitotic index is brisk and there are foci of cellular wrapping. No definite residual low grade glioma is identified in this specimen. However, the prior specimen was reviewed and showed evidence of "mixed oligoastrocytoma, WHO grade II".
