10A0 Case 10 History
Case 10 History ---- This 52 year old male was admitted for evaluation of left hemiparesis and change in personality. In the month prior to admission he had difficulty with recent memory. He had mild left hemiparesis involving face, arm, and leg was present. Tendon reflexes were hyperactive on the left. The following morning the patient was found on the floor in the bathroom unresponsive. He was treated with Decadron, hyperventilation, and Mannitol and transferred to the NICU. There was no improvement in his examination with these maneuvers. A CT scan showed a hemorrhagic lesion in the right frontal area extending into the right ventricle, surrounding edema and a right to left shift. Due to the grim prognosis, only supportive care was offered from that time. On June 10, he was found cyanotic without respirations and pronounced dead. Clinical impression at the time of death was that due to the prodromal history of personality change and progressive hemiparesis that an underlying tumor was likely with secondary hemorrhage.
