Case 2 History ---- The patient was a 74-year-old woman with a several week history of difficulty with vision, fatigue, and gait instability. Subsequent brain MRI showed a diffuse non-circumscribed T2 hyperintense process expanding the deep and subcortical white matter within the right parietal, temporal, and occipital lobes. Also present on brain MRI are FLAIR signal hyperintensities involving the posterior limb of the left internal capsule, pons, bilateral cerebral peduncles, and right middle cerebral peduncle. Patchy enhancement is also noted in multiple areas. The radiologic differential diagnosis was gliomatosis cerebri vs. encephalitis. A right-sided open biopsy of right superior temporal gyrus was performed.
