Case 12 History ---- The patient was a 62-year-old man with a glioblastoma of the right peri-sylvian fissure status post resection, radiation and temozolomide, who developed a mass in the right frontotemporal region. Operative procedure: Right frontotemporal craniotomy and resection of recurrent tumor with biopsy and IMRI. Prior analysis of material generated at another hospital performed at BJH demonstrated a GBM, IDH1 (pR132H) negative by immunohistochemistry. ---- 12A1-3 The tumor is a high-grade diffuse glioma predominantly with a fibrillary pattern. The tumor cells have atypical hyperchromatic nuclei, occasional prominent nucleoli, and moderate amounts of eosinophilic cytoplasm. Mitotic activity is brisk. Microvascular proliferation is identified. An area (arrow, 12A1) within the tumor shows elongated sarcomatous change.
