Diagnosis: The collective morphologic and immunohistochemical findings, in conjunction with imaging findings, are consistent with glioblastoma, IDH1 (R132H)-wildtype by immunohistochemistry, with sarcomatous differentiation, MGMTp unmethylated, CNS WHO grade 4.
Comment: The initial examination of this case was performed at the Michigan Medicine Laboratories (Ann Arbor, MI). They found that ATRX was retained in tumor cells; however, this immunostain was not available for our review. The resection specimen was negative for MGMT promoter (MGMTp) methylation. Chromosomal microarray was positive for polysomy 7 and monosomy 10 and homozygous CDKN2A/B deletion, and negative for EGFR amplification.
