23A1 Gliosarcoma (Case 23) H&E 2X
History ---- The patient is a 49yo male with a history of glioblastoma of the temporal lobe resected 3 times previously. Operative procedure: Right craniotomy and anterior temporal resection of tumor. ---- 23A1-12 The same microscopic fields are shown at several magnifications stained with H&E (23A1,4,7,10), for GFAP (23A2,5,8,11) and Reticulin (23A3,6,9,12). Comparison within the same magnification shows GFAP-immunoreactive high grade glial elements are separate from sarcomatous areas. Although it is a generalization, spindled elements (S, 23A10) are associated with sarcomatous reticulin deposition; and more cytoplasm rich glial elements (G, 23A10) with GFAP immunoreactivity. The cells are pleomorphic with variably sized nuclei and both populations exhibit frequent mitoses. There is prominent necrosis and microvascular proliferation.
