20A1 Oligodendroglioma, anaplastic Grade 3 (Case 20) H&E 20X 1
Case 20 History ---- The patient is a 37-year-old man with a history of anaplastic oligodendroglioma, IDH mutant, 1p19q co-deleted (original nomenclature as glioblastoma in 2006, revised diagnosis in 2019), and underwent subsequent chemoradiation. A recent biopsy (with interval laser ablation) showed scattered IDH1 mutation positive cells. Follow up images show interval increase in FLAIR signal and nodular enhancement. Operative procedure: Left craniotomy and resection of recurrent temporal lobe tumor with biopsy and possible IMRI. ---- 20A1,2 H&E stained sections show a diffusely infiltrative glioma consisting of tumor cells composed of relatively round, regular nuclei and empty cytoplasm (i.e., perinuclear halos). Prominent microvascular endothelial proliferation is present, but tumor necrosis is not appreciated. Mitoses are estimated to be up to 2-4 per 10 high power fields.
