IDH1 immunohistochemistry shows evidence for mutant tumor in all regions of the cortex and white matter, although tumor cell density varies. ---- Not shown: An immunohistochemical stain for MIB-1 highlights increased proliferative activity. The neoplastic cells are reactive for S100 protein and a GFAP stain shows focally prominent mini-gemistocytic features. A focal endothelial prominence in the vasculature is also present. There is no evidence of widespread necrosis, significant atypia or marked cellularity. However, the focal prominence of vascular endothelial proliferation and mitotic activity worrisome and merit designation as oligodendroglioma, WHO CNS Grade 3.
