The tumor was convincingly metastatic breast cancer with positivity for HER2. ---- Not shown: A panel of immunohistochemical stains were performed and show tumor cells to be diffusely and strongly reactive for CAM 5.2, EMA, and CK7; a subset are also positive for progesterone receptor. The tumor is, however, negative for GCDFP-15, CK20, synaptophysin and chromogranin. A mammaglobin immunostain shows scattered positive cells. Further workup:
ER: Positive (Allred score 5+3 = 8/8) ---- PR: Positive (Allred score 3+3 = 6/8)
---- Her-2: Intermediate (HercepTest score = 2) ---- Comment: The overall morphological and immunohistochemical findings are consistent with a metastatic poorly differentiated carcinoma. In view of her prior history, a primary from breast is favored.
