The tumor cells are negative for TTF-1. Not Shown, Tumor cells are Napsin A, and CK20 immunonegative. Overall, given the distinctive morphologic features, this tumor is most consistent with a metastatic mucoepidermoid carcinoma. Given the imaging findings of a 3.0 x 3.1 cm spiculated mass in the medial right lung apex, contiguous with the posterior medial pleural surface, this metastatic tumor is favored to represent a lung primary. Pulmonary mucoepidermoid carcinoma is known to be negative for TTF-1 and Napsin-A.
