In most areas, the Ki-67 labeling index is low (<5%), but it focally reaches up to 14%. (Ki67 IHC) ---- Not shown: CD56 shows strong and widespread tumoral immunoreactivity. There is also patchy Neu-N positivity. Rare tumor cells are positive for S-100 protein. The tumor cells are negative for epithelial membrane antigen, cytokeratin, desmin, and HMB45, and CD34. ---- Comment: The morphologic and immunohistochemical features are consistent with the diagnosis of cerebellar liponeurocytoma, WHO grade 2. Although this entity was initially assigned a WHO grade of 1, additional follow-up studies have recently shown a higher recurrence rate than initially appreciated. Therefore, in the 2007 WHO scheme, this tumor has been assigned a WHO grade 2 to reflect the relatively high risk of subsequent recurrence.
