Case 7 Denouement ---- Other immunohistochemical stains were performed. The tumor cells are non-reactive for epithelial membrane antigen, CAM 5.2, CEA, pan cytokeratin, cytokeratin 20 and cytokeratin 7. The tumor cells are negative for glial fibrillary acidic protein and neurofilament protein. Scattered cells show staining for S-100 protein, synaptophysin, C-kit and alpha fetoprotein. CD31 and CD34 highlight intratumoral blood vessels. The tumor cells are non-reactive for germ cell markers CD30, beta HCG and PLAP. The tumor cells are also non-reactive for chromogranin and NeuN. Electron microscopy showed no evidence of cilia, zipper junctions, microvilli, neurosecretory granules or filament containing tumor processes.---- Comment: We were surprised at the lack of immunoreactivity for epithelial markers in this case. Nonetheless, discussing the case with colleagues, as well as considering the patient's age, clinical and radiographic features and H&E appearance we think that this tumor is most appropriately considered a choroid plexus carcinoma, WHO grade III. This is an old case. Were it a current case we would have performed molecular diagnostic and DNA methylome studies.
