Ki-67 highlights a proliferative index focally up to 7.7%. ---- Comment: The overall findings are consistent with an atypical choroid plexus papilloma, WHO grade II. Although the mitotic activity is focally elevated to >5/10 hpfs, there is no definitive solid growth pattern (surgical coagulation mimics it) or frankly malignant features to establish a diagnosis of choroid plexus carcinoma. The presence of diffuse leptomeningeal enhancement on imaging study is concerning for leptomeningeal spread, which can rarely occur for choroid plexus papilloma. Molecular analysis showed a mutation in TERT promoter.
