Ki-67 is low, focally reaching 3%. ---- Comment: The collective features are consistent with pineal anlage tumor. Comparison with the prior patient specimen suggests that this tumor has undergone significant differentiation, likely in response to treatment. We no longer see the primitive embryonal component with high proliferation and instead see variable terminally differentiated areas with very low proliferation. Methylation profiling (NIH) confirms this tumor is consistent with a pineal anlage tumor. This case was compared to the prior pretreatment biopsy and does not show any of the primitive embryonal component that was the majority of the biopsy material. There is no appreciable mitotic activity or necrosis.
