Case 16 History ---- The patient is a 61-year-old man with a history of hypertension, type 2 diabetes mellitus, anxiety, depression, and chronic back pain, who presented with worsening back pain radiating down from his midsection to thighs and progressive gait decline secondary to pain and lower extremity weakness, resulting in falls and need for wheelchair assistance over the last two months. MRI demonstrated significant degenerative disc disease, a C8 intradural, extramedullary mass with mass effect, and moderate to severe thoracic and lumbar stenosis. The patient has bilateral cervical nerve sheath tumors and severe spinal stenoses requiring decompression, resection, and posterior spinal fusion. He was admitted to the neurosurgery service for ongoing evaluation and management of thoracic myelopathy. On 4/6, he underwent intradural tumor resection, C6-T2 posterior spinal decompression and C4-T3 posterior spinal fusion.
