Ki-67 demonstrates regionally variable proliferation that is focally as high as 18%. ---- Comment: Presence of high cellularity, increased mitotic activity, patchy p53 immunoreactivity, and partial loss of S100 protein expression are somewhat worrisome findings but given its overall small size, and relatively well-encapsulated nature as well as very much of a 'schwannoma' phenotype, we and colleagues favored all these changes to be within the spectrum of cellular schwannoma albeit with slightly elevated mitotic activity. The patient developed a new tumor/?recurrence 5 years later at T12. However, this tumor was a grade 1 schwannoma with minimal mitotic activity which showed no histologic evidence of malignant transformation.
