2A0 Case 2 History
Case 2 History ---- The patient was a 55 year old woman with a history of hypertension and hypercholesterolemia who was brought in by the emergency medical team after her husband found her unresponsive on the morning of December 4. The patient's husband stated that she was very nervous and anxious earlier that day and stated that she "thinks she has cancer." The patient was transported from home to an outside hospital where CT of the head was performed which showed a 5.5 cm massive right frontal and temporal intracerebral hemorrhage, filling the right lateral ventricle and extending into the left lateral ventricle. There was massive midline shift to the left. There is blood in the third and fourth ventricles, as well as the basal cisterns and sylvian fissures. The patient was emergently transferred to the BJH neurosurgigical intensive care unit. On arrival, she was noted to be intubated, have decreased blood pressure, fixed and dilated pupils, no corneal reflexes, no cough or gag reflex, and no response to stimulation. She was also noted to have marked thrombocytopenia and WBC greater than 100,000 with 95% blasts. The patient was pronounced dead on December 4.
