The definitive stain in this case is CD1a, establishing these neoplastic cells as Langerhans histiocytes. (CD1a IHC) ---- Comment: The presence of clusters of atypical histiocytes in the suprasellar mass of a young adult is highly suspicious for a histiocytic neoplasm. Despite the fact that only a very small amount of lesional tissue was available for immunohistochemical workup, the presence of CD1a positivity is sensitive and specific for Langerhans cell histiocytosis.
