15A0 Case 15 History
Case 15 History ---- The patient is a 66-year-old man with altered mental status and muscle weakness. Operative procedure: Left gastrocnemius muscle and left sural nerve biopsies. ---- Microscopic examination of the gastrocnemius muscle biopsy material showed striated muscle fibers with marked atrophy. Small vessels in the perimysium and endomysium are filled with large atypical and mitotically active lymphoid cells which are CD20 immunoreactive and are almost entirely limited to vascular lumina without any extension into the adjacent soft tissue. A CD34 highlights the intravascular tumor cells. Scattered admixed small lymphocytes and histiocytes are positive for CD3 and CD68 respectively.
