Case 3 History ---- The patient is a 63-year-old woman with aphasia, weakness, and a history of multiple ischemic strokes over the last 4 months. Brain MRI shows confluent areas of diffusion restriction and FLAIR abnormality in the subcortical white matter with patchy regions of contrast enhancement. The radiologic differential diagnosis included atypical posterior reversible encephalopathy syndrome, vasculitis, thrombotic microangiopathy, and reversible cerebral vasoconstriction syndrome (RCVS). Autoimmune serologies had been negative before surgery. Operative procedure: Right frontal craniotomy for open biopsy.
