Case 2 History ---- The patient is a 47 year old man with HIV/AIDS with a history of diffuse Kaposi sarcoma and chronic renal disease who presented with altered mental status and ataxia. Brain MRI showed three enhancing lesions in the right frontal, right occipital and left temporal regions. All were associated with vasogenic edema. The patient was treated empirically for CNS toxoplasmosis and lymphoma. Further FDG-PET studies showed avid FDG uptake in the brain masses consistent with CNS lymphoma. The patient’s mental status continued to decline until care was withdrawn and the patient expired. ---- At autopsy the unfixed brain weighed 1450 grams. The leptomeninges were thin and translucent in most sites. The underlying cortex did not demonstrate areas of hemorrhage, softening, or cystic tissue loss. There was no evidence of uncal or subfalcine herniation. The external surfaces of the cerebellum, brainstem and spinal cord were unremarkable.
