An angiocentric infiltration pattern is frequent with splitting of their argyrophilic fibre network seen in this characteristic reticulin pattern, even within totally necrotic areas of neoplasm. (reticulin) ---- A marked reactive astrocytosis, composed of mostly gemistocytic forms, borders the neoplasm. In addition, the tumor has induced an intense granulomatous inflammatory response consisting of large numbers of lymphocytes, plasma cells, epithelioid histocytes and giant cells, with occasional PMN's, gitter cells (i.e., lipid laden and hemosiderin macrophages). A final diagnosis of primary CNS lymphoma with extensive necrosis involving the right hypothalamus, and, focally, basal ganglia and internal capsules (bilaterally), optic chiasm and right optic tract.
