Case 24 History ---- The patient is an 81 year old man with two enhancing hemispheric brain lesions. Clinical differential diagnosis: Lesion differential is CNS lymphoma versus metastatic disease versus primary glioma. Operative procedure: Stereotactic brain biopsy of first lesion. ---- 24A1,2 Histologic sections show brain parenchyma with prominent perivascular aggregates of abnormally large, irregular, discohesive lymphocytes with nuclear pleomorphism, high nuclear to cytoplasmic ratio, and prominent nucleoli. These atypical lymphocytes also infiltrate the parenchyma, and are admixed with a smaller population of benign-appearing, small mature lymphocytes.
