Case 2 History ---- The patient was a 69 year old female admitted in 1/2002 for abdominal pain and distension caused by a perforated colon and peritonitis. Her past medical history was significant for a right lung transplant 7/2001 due to idiopathic pulmonary fibrosis. She had been diagnosed with CMV colitis and post-transplant lymphoproliferative disease (PTLD, B cell). Her hospital course was complicated by TTP/HUS and acute renal failure. She became progressively obtunded and finally comatose. EEG demonstrated temporal spike activity that was treated with valproic acid. She remained comatose and, per the family's request, she was given comfort care only and expired 3/2002.---- At autopsy the weight of the unfixed brain is 1,230 gm. The histomorphologic features and immunohistochemical profile are those of a large cell lymphoma of B-cell phenotype. ---- 2A The orbital surface of the left frontal lobe and inferior portion of the left cerebellum (arrow) appear cloudy
