Case 5 History ---- The patient is a 69-year-old man who presents with a three month history of back pain, a 3-4 day history of lower extremity weakness and gait difficulty with evidence of myelopathy on physical exam and imaging studies demonstrating a lytic lesion at T9 as well as L5. Operative procedure: Posterior lumbar spinal fusion, decompression. ---- 5A1,2 Sections of the T9 epidural mass show a hypercellular proliferation of monomorphic appearing epithelioid cells growing in a sheet-like architecture with a moderate amount of eosinophilic to amphiphilic cytoplasm, eccentrically placed, round nuclei with scattered "clock face" chromatin. The tumor shows numerous mitotic figures.
