This area transitions into even more aggressive appearing tumor with a predominantly sarcoma-like histology. In these areas, there is variable cellularity, with the majority of tumor cells being spindled and arranged in fascicles. Some cells have a strap like appearance with occasional suggestions of cross striations. In this sarcoma-like component, the mitotic index is markedly elevated and the tumor appears frankly anaplastic, given that it no longer has recognizable meningothelial features. Per outside report, the mitotic index reaches up to 18 mitoses per 20 high power fields. The histology of the recurrent neoplasm in 2008 is entirely sarcoma-like and mostly resembles fibrosarcoma. This is similarly consistent with the diagnosis of anaplastic meningioma, WHO grade III.
