Ki67 identifies proliferative activity which may not be significantly increased. In this case Ki-67 is moderate, up to 10.5% by manual count. ---- Not shown: Atypical features of this meningioma including necrosis, hypercellularity, focal nucleoli, and small cell change are present. Scattered mitoses are present, up to 4 per 10 HPF. ---- Comment: The collective features are consistent with atypical meningioma, CNS WHO grade 2. This tumor meets criteria for grade 2 by mitotic count, by brain invasion, and by >3 of 5 atypical histologic features.
